Back to the original question....I asked my wife about this yesterday......

The phrase can also be used when someone is slightly embarrassed about something....

For example, you buy someone a small gift. She is a bit embarrassed to take it. Or, you and your girlfriend are eating dinner and she spills a glass of water on your pants. Or your novia in Colombia is a little short on the bills this month and asks to borrow $50 (which is very difficult for a Colombiana with good intentions).

So, as you can see, the phrase does not always mean something negative. As with English, phrases must be understood in context....which is what makes another language so difficult for those trying to learn it. I learn something new every day.
